2008 Spring Corporate Support Opportunities
IMPORTANT
  • The deadline for submitting Corporate Support Requests for the Spring Conference is Friday, May 9, 2008.

  • Want to change your corporate support? - You have already submitted a support request, and want to make changes or add items to your request, select the revise option. Entering a completely new form will erase your previous choices.

  • Returning to submit a form for another organization? - Your request is connected to your UserID. If you are submitting requests for multiple organizations, you MUST have a user profile in the database for each organization.

  • Need help? Contact support@oasfaa.org.

  • This is a multi-part form and is not complete until you reach the final invoice page.
  • Login
    I AM NOT in the OASFAA database
    Your next screen (Part 2) will be a blank Member Profile page. Select a password. Do not use personal information for your password since this will be emailed to you. You will receive a copy of your Username and Password by email, following your initial login. Use these for any future logins. Complete the form and click "Continue" to go to Part 3.

    I AM in the OASFAA database
    Fill in your Username and Password. Your next screen (Part 2) will be your Member Profile as it currently appears in the database. You may change your password and make any other necessary corrections. Click "Continue" to go to Part 3.
  • Revise Your Request
    If you are returning to revise your request , please use the "Revise Existing Request " section below. Enter your Username and Password. Part 2 is your Member Profile. In Part 3, your previous selection(s) will be shown. Make any necessary changes. Your previously entered information will be deleted and replaced with the new.
  • Cancel Your Request
    Canceling your request? Select "Cancel Request " and enter your Username and Password. Your sponsorship will be request from the records and cannot be recovered.
NOTE: Please be aware that your unique username and password are linked to both you and your submission.
